Sauté the Sausage:
In a large skillet, heat olive oil over medium heat. Add onion and garlic, sauté until fragrant (about 2 minutes).
Add sausage chunks and cook until browned and fully cooked.
Make the Sauce:
Lower heat, stir in heavy cream and Parmesan cheese. Mix until cheese melts and sauce is creamy. Add a bit of reserved pasta water if needed.
Combine:
Add the cooked pasta and toss to coat. Stir in fresh spinach and cook for 1–2 more minutes, until wilted.
Season & Serve:
Season with salt, pepper, and chili flakes if desired. Serve hot with extra Parmesan on top.
